The patient's admitting diagnosis must be printed in box 69 on the UB04 institutional claim form. DrChrono makes it easy to add to your patient's claim.
- Navigate to Billing > Live Claims Feed > inside patient's appointment > Line items transactions section
- The admitting diagnosis code is added to the "Adm" box highlighted below. The diagnosis can be added by code or looked up using text.
- The diagnosis added to this box will be listed as the admitting diagnosis code in box 69 of the UB04 form.
