UB04 Box 26 - Utilizing a claim ID from a claim form

Each patient appointment within DrChrono will be assigned a unique claim ID number automatically. This number will be transmitted to the payer with each submitted claim.

On the HCFA-1500 form, it will print in box 26 under the label "Patient's Account No.". The first 6 digits will be your client group account number with DrChrono and the following 9 digits are the patient's claim ID/account number.

  • Blue box - The first 6 digits are your DrChrono client group account number
  • Green box - The last 9 digits are your patient's claim ID/account number

The patient's claim id/account number can be entered into the box labeled "drc claim #" in your Live Claims Feed. It will pull up the patient and the exact date of service.

You can also view these claim id/account numbers in the body of your Live Claims Feed if you pull up your patient by name, date of service, or other identifiers.

On the UB04 form, it will print in box 3B under the label "Med. Rec. #".
