Date of Current Illness vs Initial Treatment date; what is the difference?

Many different scenarios would require a specific date (other than the date of service) to be listed on the claim form.

Two of the most common are the Date of Current Illness, Injury, or Pregnancy and the Initial Treatment date. Below will show you how to help ensure the dates shown on your claims so they are accepted and processed by the payer without delay.

Date of Current Illness, Injury, or Pregnancy (LMP)

Description: Identifies the first date of onset of illness, the actual date of injury, or the LMP for pregnancy in box 14 on the HCFA 1500 form. (Source: NUCC, National Uniform Claim Committee)

  • Related qualifiers include:
    • 431 - Onset of Current Symptoms or Illness
    • 484 - Last Menstrual Period

Where to enter this info in DrChrono -

  • In the Live Claims Feed - this information can be entered in the right column. The first box will allow you to select the qualifier and the second box will allow you to enter the date (MMDDYYYY)

Initial Treatment

    • 444 - Initial Visit Date
    • 454 - Initial Treatment Date
    • 304 - Last Related Visit
